Here are 4 Basic Steps to Making a Good Work Accident Investigation Report:

Get Relevant Facts

After we make sure the room where the workplace accidents are safe, we must put all the facts that are related to the accident. As an example:

  • The specific date, time, and place of the accident
  • Name of victim, position, department, and direct supervisor
  • Name and personal data of several witnesses
  • The events before the accident
  • What specific task was the victim working on at that time
  • Environmental conditions
  • The existing situation
  • The resulting injuries
  • Types of wound care
  • Crash reconstruction pictures
  • Damage to equipment, materials, and so on

Set the Sequence of Events

Based on the evidence you have, you should be able to sort the events from the time the accident occurs. On your workplace investigation checklist, describe this position in detail including:

  • Events that resulted in accidents
  • The incident during the accident
  • The incident immediately after the accident
  • What do employees do? Grasping his knees, clutching his elbows, closing the wound, screaming.

Accident Analysis
